FAMILY MEDIATION, PSYCHOLOGICAL TESTING AND FORENSIC ASSESSMENT/CONSULTATION
	Dr. Street's rate for Child Custody Evaluations (CCEs) $225.00 per hour. A Court Order and a retainer of $6,000.00 are required prior to the start of a Child Custody Evaluation. Depositions related to CCEs are billed at the rate of $350.00 per hour. Testimony in Court related to CCEs will be billed at the rate of $2,250.00 per half day and $4,500.00 per full day of scheduled time in court. All fees related to depositions and testimony must be paid two weeks in advance of the scheduled deposition or court testimony. Forensic Assessments are billed at $150.00 per hour and a retainer of $1,500.00 is required prior to the start of the assessment. Family Mediation sessions are $150.00 per hour payment is required at time of service. Payment is due at the beginning of each session.
	INDIVIDUAL THERAPY: ADULTS AND ADOLESCENTS
	Dr. Street offers a free 30-minute consultation session so that she and potential clients can assess the goodness of fit between them.  All appointments thereafter are $125 per 45 minute session. Payment is due at the beginning of each session.
	The following payment methods are accepted: cash, check, Debit, Visa, MasterCard, American Express and Discover.
	INSURANCE COVERAGE
	Dr. Street does not work with insurance companies. However, you may contact your insurance agency to ask about "out-of-network" coverage. If your insurance offers this type of coverage, you may choose to submit invoices to your insurance company for reimbursement. It is recommended that you contact your insurance provider after your consultation session and ask about benefits for psychotherapy.
	CANCELLATION POLICY
	Dr. Street has a 48-hour cancellation policy. You will be billed for missed appointments that are not cancelled at least 48 hours prior to the scheduled time.
